  • Who is involved in issue management? Who is accountable for ensuring issues are addressed and closed?
  • How are you connecting issue management into other ONFR framework components? For example, linking to events/incidents, incorporation into RCSA, consideration when setting risk appetite.
  • How does issue management support proactive risk management?
  • Does your firm perform any trend analysis on your data relating to issues? For example, concentration of issues by risk or process, the time of year at which most issues are raised.
  • Have you identified opportunities to incorporate AI into your issue management approach? For example, by automatically identifying or logging issues?
  • What does your firm see as the greatest challenges associated with issue management? What, if anything, are you changing/planning to change to address these?
